Custom Design Intake & Policy Agreement

Welcome! We are excited to bring your custom apparel vision to life. To ensure a smooth, efficient design process and to deliver exactly what you are looking for, please complete this intake form thoroughly. Please review our design policies below before signing.


1. Our Custom Design Policy & Boundaries

• Design Deposit: A non-refundable design deposit of $30.00 is required before any design work begins. This deposit will be credited entirely toward your final production invoice if you proceed with the order.

• Initial Concepts: Based on the details provided in this form, you will receive up to two (2) distinct initial design concepts to choose from.

• Included Revisions: Your design includes up to two (2) rounds of minor revisions (such as text adjustments, font changes, or color tweaks) on your chosen concept.

• Additional Revisions: Any additional revision rounds beyond the first two, or requests for a complete redesign from scratch, will incur a fee of $20.00 per round.
